Before your first visit, contact your insurance carrier to confirm:
If the patient’s insurance accepts a super bill, the patient can submit this directly to insurance for a claim for the charges normally covered by insurance (ie. $225 first office visit and $160 return office visits). Please ask for a copy of the super bill at the time of scheduling.
If the doctor is not contracted with your insurance company – and for all services not covered by insurance – payment is due at the time of service.
